Abstract Objective: To investigate the effectiveness of varicocelectomy to treat varicocele of patients with different ages. Methods: From September 2008 to June 2015, 130 patients with varicocele and idiopathic infertility had came to Wuyi county hospital of traditional chinese medicine for varicocelectomy were recruited in this prospective study. And all patients were divided into 35 years old or younger group (group 1) and older than 35 years (group 2). Semen parameters of all patients were compared between varicocelectomy before and after. Spouse pregnancy rate of patients also were compared between the two groups. Results: The mean sperm concentration of patients after varicocelectomy in both groups had increased significantly (P< 0.05). The percentage of motile sperm of patients equal to or higher than Class B in group 1had increased from 47.6±5.5 to 56.6±7.2 after varicocelectomy (P<0 .05). The percentage of motile sperm of patients equal to or higher than Class B in group 2 had increased from 46.5±5.4 to 54.3±6.4 after varicocelectomy (P<0 .05). The percentage of sperm with abnormal morphology of patients in group 1 had decreased from 62.7±4.3 to 59.3±5.8 after varicocelectomy (P<0 .05). The percentage of sperm with abnormal morphology of patients in group 2 had decreased from 61.6±5.9 to 58.5±7.2 after varicocelectomy (P<0 .05). However, there was no statistical significant difference in the percentage of motile sperm of patients equal to or higher than Class B and percentage of sperm with abnormal morphology between the two groups (P > 0.05). Spouse pregnancy rate of patients had no significant difference between the two groups (P=0.75). Conclusion: For patients with different ages, varicocelectomy has the similar effect on improving semen parameter and spouse pregnancy rate.
