Clinical diagnostic efficacy of the combined transvaginal and transabdominal ultrasonography for the ectopic pregnancy |

Abstract To analyze the clinical diagnostic values of the transvaginal ultrasound, the transabdominal ultrasound, and the combined transvaginal and transabdominal ultrasonography for the ectopic pregnancy. Methods: The clinical data of 110 patients with the suspected ectopic pregnancy admitted to hospital from June 2020 to June 2023 were selected as study subjects. All these patients had undone pathological examination and the examinations by transvaginal ultrasound and transabdominal ultrasound. The diagnostic results and diagnostic coincidence rate for the different types of ectopic pregnancy and the image features of the patients with different types of ectopic pregnancy were compared between transvaginal ultrasound examination and the transabdominal ultrasound examination. Receiver characteristic curve (ROC) was used to analyze the diagnostic value of the examinations of the transvaginal ultrasound and transabdominal ultrasound for the ectopic pregnancy of the patients. Results: There were 6 patients with lost to follow-up, and there were 104 patients participated in the analysis finally, including 95 patients with the positive ectopic pregnancy confirmed by pathological diagnosis and 9 patients with the negative ectopic pregnancy confirmed by pathological diagnosis. The positive detection rate of the ectopic pregnancy of the patients by the transvaginal ultrasound was 75.8%, and the negative detection rate was 22.2%. The positive detection rate of the ectopic pregnancy of the patients by the transabdominal ultrasound was 77.9% and the negative detection rate was 22.2%. The positive detection rate of the ectopic pregnancy of the patients by the transvaginal ultrasound combined with the transabdominal ultrasound was 89.5%, and the negative detection rate was 88.9%. The positive and negative detection rates of the transvaginal ultrasound combined with the transabdominal ultrasound for the ectopic pregnancy of the patients were significantly higher than those of the transvaginal ultrasound or the transabdominal ultrasound alone (all P<0.05). The diagnostic coincidence rates and the detection rates of accessory mass, intrauterine false gestational sac, pelvic effusion, heart tube pulse, embryo, fetal heart and yolk sac of the transvaginal ultrasound combined with the transabdominal ultrasound of the patients were significantly higher than those of the transvaginal ultrasound or the transabdominal ultrasound alone (P<0.05). The area under the curve (AUC) of the transvaginal ultrasound combined with the transabdominal ultrasound of the patients for diagnosing their ectopic pregnancy was 0.892, and which was significantly higher than that (0.510) of the transvaginal ultrasound and that (0.501) of the transabdominal ultrasound (P<0.05). Conclusion: The diagnostic value of the combined transvaginal ultrasound and transabdominal ultrasound examinations for the ectopic pregnancy of the patients is higher.
