Abstract To investigate the effects of combination of spleen and stomach ten-eedle acupuncture and acupoint suture embedding combined with antagonist fixation protocol for treating patients with diminished ovarian reserve (DOR) on their granulosa cell apoptosis and their pregnancy outcomes after in vitro fertilizationembryo transfer (IVF-T). Methods: A total of 82 patients with DOR who planned to undergo IVF-T were selected and were divided into two groups (41 cases in each group) according to the random number table method. The patients in the control group were treated with antagonist fixation protocol, while the patients in the observation group were treated with the combination of spleen and stomach ten-eedle acupuncture and acupoint suture embedding combined with antagonist fixation protocol. The clinical efficacy, TCM syndrome score, serum reproductive hormone level, granulosa cell apoptosis, ovulation induction, and pregnancy outcomes after IVF-T of the patients in the two groups before and after treatment were observed. Results: The total effective rate (92.7%) of the patients in the observation group was significantly higher than that (75.6%) of the patients in the control group (P<0.05). After treatment, the irregular menstruation score, limb fatigue score, loss of appetite score, chest distension, and tightness score, loose stool score of the patients in both groups had decreased significantly, and which of the patients in the observation group were significantly lower than those of the patients in the control group (P<0.05). After treatment, the serum estradiol (E2) level of the patients in both groups had increased significantly, and the levels of follicle stimulating hormone (FSH) and luteinizing hormone (LH) of the patients in both groups had decreased significantly. The levels of E2 (178.23±40.32 pmol/L), FSH (9.82±1.06 U/L), and LH (5.52±0.66 U/L) of the patients in the observation group after treatment were significantly better than those (160.93±37.17 pmol/L, 12.33 ± 2.07U/L, and 7.03 ± 0.71 U/L) of the patients in the control group (P<0.05). There were no significant differences in the dose and days of Gn used for ovulation induction of the patients between the two groups (P>0.05). The egg obtained rate and MII egg rate of the patients in the observation group were significantly higher than those of the patients in the control group (P<0.05). The apoptosis rate of granulosa cells (24.3±9.2%) of the patients in the observation group was significantly lower than that (33.8±12.2%) of the patients in the control group (P<0.05). The implantation rate of embryo (54.6%), the clinical pregnancy rate (78.0%), and the live birth rate (87.5%) of the patients in the observation group were significantly higher than those (39.1%, 53.7%, and 68.2%) of the patients in the control group (P<0.05). Conclusion: The combination of spleen and stomach ten-eedle acupuncture and acupoint suture embedding combined with antagonist fixation protocol for treating the patients with DOR after IVF-T has positive influence on their pregnancy outcomes, which can significantly improve the embryo implantation rate and the clinical pregnancy rate, and can reduce the apoptosis of granulosa cells.
