Effect of cumulus granulosa cell and interleukin-6 and C-reactive levels on the clinical outcomes in the infertile women with endometriosis after IVF-ET |

Abstract To investigate the effect of cumulus granulosa cell mass and the changes of interleukin-6 (IL-6) and C-reactive protein (CRP) levels on the clinical outcomes of the women with endometriosis (EMS) after in vitro fertilization-embryo transfer (IVF-ET). Methods: The clinical data of women after IVF-ET from January 2018 to January 2020 were collected. The infertility women with EMS were in study group and the women with infertility cause by her husband were in control group. The baseline data, such as age, infertility duration, and body mass index (BMI), the sinus follicle number, the levels of serum follicle-stimulating hormone (FSH), luteinizing hormone (LH) and estradiol (E2), the IL-6 and CRP levels in follicular fluid, and pregnancy outcomes of the women were compared between the two groups. Annexin V - FITC/PI method was used to detect the apoptosis rate of cumulus granulosa cell in follicular fluid. The correlation of inflammatory factor levels in follicular fluid of the women with EMS and the apoptosis rate of germ hillock granulosa cell with their outcomes after IVF-ET was analyzed by Pearson analysis. Results: The levels of IL-6 (364.23±48.53 ng/L) and CRP (10.73±0.48 ng/ml) in follicular fluid and the apoptotic rate of cumulus granulosa cells (25.2±3.4) % of the women in the study group were significant higher than those of the women in the control group, but the fertilization rate (64.2±5.4%), egg maturation rate (65.8±5.6%), and clinical pregnancy rate (33.3%) of the women in the study group were significant lower (all P<0.05). The levels of IL-6 and CRP in follicular fluid, and the apoptotic rate of cumulus granulosa cells of the women with EMS were negatively correlated with their fertilization rate, egg maturation rate, and clinical pregnancy rate (P<0.05). Conclusion: The clinical outcomes of the women with EMS after IVF-ET are poor, which is closely to the levels of IL6 and CRP in follicular fluid and the apoptotic rate of cumulus granulosa cells. So the levels of IL-6 and CRP in follicular fluid and the apoptotic rate of cumulus granulosa cells may be used to predict the clinical outcomes of the women after IVF-ET.
