Clinical significance of detection index of intrahepatic cholestasis of pregnancy for diagnosing fetal distress |

Abstract To discuss clinical significance of detection index of intrahepatic cholestasis of pregnancy (ICP) for diagnosing fetal distress, and to analyze receiver-operating characteristic (ROC) curve. Methods: From June 2013 to June 2016, the total 78 pregnant women with ICP in maternity and child health care hospital of Fuyang district were recruited. Among them, 31 women with intrauterine fetal distress were included in observation group; the other 47 women without intrauterine fetal distress were included in control group. At the same time, 30 healthy pregnant women were selected into healthy control group. The level of total bile acid (TBA) and cholylglycine (CG) were compared among the three groups. And the parameters of umbilical artery, such as ratio of systolic and diastolic bloodstream (S/D) of umbilical artery, value of resistance index (RI) and volatility index (VI) were also compared among the three groups. ROC curve was used to statistical analysis for trying to find the best diagnostic indicators. Results: The level of TBA and CG of women in control group was significant higher than that of women in observation group, and that of women in observation was significant higher than that of women in health control group (P< 0.01). S/D and value of RI of women in control group was significant higher than that of women in observation group, and that of women in observation was significant higher than that of women in health control group (P< 0.01). Value of VI had no different among the three groups (P> 0.05). ROC curve analysis showed that AUC area of joint parameters (TBA, CG, S/D) was 0.969, the sensitivity was 92.9% and specificity was 93.7%. Conclusion: Level of TBA, CG and S/D will increase in ICP women with fetal distress. Above parameters combined can be used as the best diagnostic criteria to prevent fetal distress.
