Abstract Objective: To explore the execution of the standard operation of intrauterine device (IUD) insertion in family planning institutes at the grassroots level for further exploring the clinical pathway of the standardized technical operation in IUD insertion. Methods: The scenario analysis method recommended by the American Population Council was used to collect relevant data and information about the standard operation of IUD insertion. Results: The scores of knowledge on disinfection, clinical knowledge, regular knowledge but basic knowledge among the county family planning personnel in the theoretical examination of service providers were significantly higher than those among the township family planning personnel (P<0.05). The scores of auxiliary examination rooms, operating rooms, surgical quality management level, well-equipped rescue drug among the county in surgical quality management and assessment of the hardware were significantly higher than those among the township (P<0.05), while there was no significant difference in the management of the sterile items (P>0.05). There were no significant differences in scores of outpatient records and preoperative consent form in the operation process of IUD insertion between the two groups (P>0.05). In the aspect of preoperative medical history collection, the physical examination and personal hygiene, brush in hand before surgery, sterilization techniques, operation process of IUD insertion, surgical records and follow-up arrangements, the scores of the county was significant higher than those of the township. Conclusion: Overall, although the standard operation level of IUD insertion in grassroots family planning institutes is higher in the county than in the township, operation levels of the two service agencies should be further strengthened.
