Effect of midwifery management method based on comprehensive systematic evaluation for reducing the rate of episiotomy of primipara |

Abstract To analyze the effect of midwifery management method based on comprehensive systematic evaluation for reducing the rate of episiotomy of primipara. Methods: 540 primiparas who gave birth vaginally during the traditional management methods adopted by obstetrics department from January to December 2021 were selected in control group, and 726 primiparas who gave birth vaginally during the midwifery management method based on comprehensive systematic evaluation adopted by obstetrics department from January to December 2022 were selected in experimental group.The perineal conditions and the postpartum indicators of the primiparas were compared between the two groups. Results: The lateral episiotomy rate (19.7%) of the primiparas in the experimental group was significantly lower than that (80.4%) of the primiparas in the control group (P<0.05). The perineal laceration rate (24.9%) of the primiparas in the experimental group had no significantly different from that (23.9%) of the primiparas in the control group (P>0.05). The postpartum blood loss (139.5±26.7ml) and the hospital stay (3.7±1.1d) of the primiparas in the experimental group were significantly less than those (146.9±29.3ml and 4.1±1.3d) of the primiparas in the control group, and the score (3.46±1.02 points) of Numerical Rating Scale (NRS) of the primiparas in the experimental group in the postpartum 24h was significantly lower than that (4.60±1.37 points) of the primiparas in the control group (all P<0.05). Conclusion: The midwifery management method based on comprehensive systematic evaluation for the primipara during vaginal delivery can reduce the rate of lateral episiotomy, relieve the postpartum pain, and decrease the postpartum hemorrhage and hospitalization time.
