Analysis of different diagnostic criteria for diagnosing pregnant women with thyroid diseases |

Abstract Objective: To investigate the value of different diagnostic criteria for diagnosing pregnant women with thyroid diseases. Methods: From January 2016 to January 2018, The pregnant women accepted initial thyroid function tests were selected into this study, which included 469 women (1-12 gestational weeks) in group A, 536 women (13-27 gestational weeks) in group B, and 301 women (28-40 gestational weeks) in group C. The serum TSH, FT4 levels of all included women were detected by American academy of clinical biochemistry (NACB) standard and by reference range of local diagnostic standard. Results: The incidence of thyroid disease of pregnant women diagnosed by NACB in group A, B, and C was 23.0%, 38.8%, and 52.9%, respectively, which were significant higher than those(7.7%, 8.0%, 6.3%, respectively) of women diagnosed by reference range of local diagnostic standard in the three groups. In group B and C, the incidence of thyroid disease of women over 30 years old diagnosed by reference range of local diagnostic standard were 14.8% and 17.2%, which were significant higher than those (6.6% and 1.8%) of women less than 30 years old. The incidence of thyroid disease of multipara in group B and C were 10.5% and 23.1%, which were significant higher than those (6.1% and 5.6%) of the first parturient women (P<0.05). Conclusion: The diagnostic criteria for diagnosing thyroid diseases of local by ATA standard maybe have the problem of over diagnosed, so it is very necessary for establishment of reference range of local diagnostic standard for clinical diagnosis and treatment thyroid diseases of pregnant women. The thyroid diseases screening should be strengthened for multipara and pregnant women over 30 years old.
