Abstract The insulin-like growth factors (IGFs) signaling system is one of the key pathways that regulating the growth, development, and aging of the body. The signaling of IGFs is regulated by the insulin-like growth factor-binding proteins (IGFBPs). IGFBP5, as a member of the IGFBPs family, has the most conserved sequences among the members of the IGFBPs family across species yet, and which has the most diverse biological roles. The expression level and the functional role of IGFBP5 may differ in the different contexts. The results of several previous studies show that the elevated level of IGFBP5 may reduce the reproductive capacity of the body. However, the results of the current studies on the influence of the elevated IGFBP5 level of the body on their disorders of glucose-lipid metabolism and their development of diabetic complications are all still highly contradictory. In conclusion, IGFBP5 is a multifunctional protein with the strong background specificity in its effect and mechanism of action, which is capable of the exerting positive or negative regulatory effect in a specific space and time, through different pathways.
