Value of four-dimensional ultrasound combined with the serum circular RNA homeodomain-interacting protein kinases 3 and circular RNA 003416 of pregnant women for diagnosing the congenital heart disease of their fetuses |

Abstract To explore the value of four-dimensional ultrasound combined with the levels of serum circular RNA homeodomain-interacting protein kinases 3 (circHIPK3) and circular RNA 003416 (circ0003416) of pregnant women for diagnosing the congenital heart disease (CHD) of their fetuses. Methods: The clinical data of 324 pregnant women with the suspected of fetal CHD from November 2021 to November 2023 were collected. These women were divided into study group (43 fetuses with CHD) and control group (281 fetuses without CHD) based on the postpartum follow-up results or the pathological results after induced abortion as the gold standard. The general data of the women in the two groups were collected. Real-time fluorescence quantitative PCR (qRT PCR) method was applied to detect the levels of serum circHIPK3 and circ0003416 of the women in the two groups. The results of four-dimensional ultrasound examination of the women in the two groups were recorded. Kappa was used to analyze the consistency between the four-dimensional ultrasound combined with the levels of circHIPK3 and circ0003416 of the women and the gold standard for diagnosing the fetal CHD. Receiver operating characteristic (ROC) curve was used to analyze the diagnostic value of the four-dimensional ultrasound combined with the levels of circHIPK3 and circ0003416 of the women for their fetal CHD. Results: The serum circHIPK3 level (1.41±0.18) of the women in the study group was significantly higher than that (1.19±0.13) of the women in the control group, and the serum circ0003416 level (0.82±0.11) of the women in the study group was significantly lower than that (1.03±0.14) of the women in the control group (all P<0.05). The consistency between the four-dimensional ultrasound of the women for diagnosing their fetal CHD and the final diagnostic result by the gold standard was better (Kappa=0.809, P<0.001). The area under the curve (AUC) of the four-dimensional ultrasound, the serum circHIPK3 level or the serum circ0003416 level of the women for diagnosing their fetal CHD was high. The AUC and the sensitivity of the combined four-dimensional ultrasound and the levels of serum circHIPK3 and circ0003416 of the women for diagnosing their fetal CHD were 0.976 and 97.7%, and which were the highest (P<0.001). Conclusion: The maternal serum circHIPK3 level of the women with fetal CHD increases, while their serum circ0003416 level decreases. The combination of the four-dimensional ultrasound and the levels of serum circHIPK3 and circ0003416 levels of the pregnant women for their fetal CHD has higher diagnostic value, which can provide the references for the clinical screening of CHD.
