Abstract To analyze the risk factors of pre-pregnancy underweight, overweight or obesity of women of reproductive age in rural areas of Henan province from 2013 to 2018. Methods: A total of 3 686,404 women of reproductive age from the rural areas of Henan province who had participated in the "National free prepregnancy health examination project" from 2013 to 2018 were selected as the research objects. Multi-factor logistics regression model was used to analyze the risk factors of the pre-pregnancy underweight, overweight and obesity of the women. Results: The high education level (college/university or above, OR=1.34, 95%CI 1.28-1.40), the vegetarian diet (OR=1.06, 95%CI 1.01-1.10) and the passive smoking (OR=1.22, 95%CI 1.20-1.24) of the women were the risk factors for their pre-pregnancy underweight. The history of pregnancy and birth, the ethnic minorities (OR=1.06, 95%CI 1.03-1.10), the vegetarian diet (OR=1.16, 95%CI 1.13-1.20), the smoking (OR=1.28, 95%CI 1.17-1.41) and the passive smoking (OR=1.04, 95%CI 1.03-1.06) of the women were the risk factors for their pre-pregnancy overweight. The history of pregnancy and birth, the vegetarianism (OR=1.15, 95%CI 1.09-1.22) and the smoking (OR=1.69, 95%CI 1.47-1.93) of the women were the risk factors for their pre-pregnancy obesity. For each additional year of age of the women, the risk of their pre-pregnancy overweight increased by 3% and their risk of pre-pregnancy obesity increased by 2%. According to the geographical location, the western area of Henan province was a risk factors of the pre-pregnancy underweight (OR=1.65, 95%CI 1.63-1.68) of the women of reproductive age, and the northern area of Henan province was the risk factors of the pre-pregnancy overweight (OR=1.09, 95%CI 1.08-1.10) and the pre-pregnancy obesity (OR=1.26, 95%CI 1.24-1.28) of the women of reproductive age. Conclusion: The women of reproductive age in Henan province should be paid attention to the problem of their abnormal pre-pregnancy weight, strengthen their weight management, given intervene of their pre-pregnancy weight rationally, and avoid the risk factors that may cause their pre-pregnancy underweight, overweight and obesity, so as to reduce the occurrence of the adverse pregnancy outcomes of the women, and improve the reproductive health of the women of reproductive age.
