Evaluation value of ultrasounic results for the delivery mode for patients with the second stage of labor exceeding two hours |

Abstract To analyze the value of the ultrasounic results of parturients with the second stage of labor>2h for evaluating their delivery mode of the parturients. Methods: The clinical data of 614 parturients with the second stage of labor>2h who had accepted two dimensional ultrasound examination during hospital delivery from January 2021 to January 2022 were selected in this the study. The delivery process, the delivery mode, and the pregnancy outcomes of these parturients were recorded. The delivery duration and the delivery mode were compared among the parturients with different ultrasound characteristic during labor. The predictive value of the indicators examined by ultrasonic detection during delivery of the parturients for their natural delivery was analyzed. Results: The angle of progression (AOP) of the parturients with cesarean section was significantly lower than that of the parturients with natural delivery, and the proportion of head perineal distance (HPD) of the parturients with cesarean section was significantly higher than that of the parturients with natural delivery group. The durations of the parturients with AOP>135°and HPD≤4cm during cesarean section were 56.98 min and 13.68 min, and durations of the parturients with AOP>135°and HPD≤4cm during were 52.79 and 13.95 min, which were significantly lower than those (103.55 and 20.11min, and 101.35 and 22.16 min) of the parturients with AOP≤135°and HPD≤4cm. The natural delivery rates of the parturients with AOP > 135 (100.0%) and HPD <4cm (100.0%) were significantly higher than those (81.2% and 84.0%) of the parturients with AOP ≤135° and HPD >4cm (all P<0.05). The sensitivity, the specificity, and the best cutoff value of the AOP value of the parturients for predicting their natural delivery were 83.6%, 86.5%, and 124.78°, respectively. The sensitivity, the specificity, and the best cutoff value of the HPD value of the parturients for predicting their natural delivery were 79.2%, 80.9%, and 4.61cm, respectively. Conclusion: The ultrasounic indexes, such as AOP and HPD, of the parturients are helpful to evaluate their delivery mode and their duration of pregnancy, and which are worth to be used more widely in clinical.
