Effect of ergonovine maleate combined with carboprost tromethamine for treating women with postpartum hemorrhage due to uterine atony |

Abstract To investigate the effect of ergonovine maleate combined with carboprost tromethamine for treating women with postpartum hemorrhage due to uterine atony, and to study its influence on fibrinolysis and coagulation of these women. Methods: The clinical data of 98 women with postpartum hemorrhage due to uterine atony from January 2018 to June 2022 were analyzed retrospectively. According to the treatment method, these women were divided into group A (42 cases with single ergonovine maleate or carboprost tromethamine of treatment) and group B (56 cases with ergonovine maleate combined with carboprost tromethamine of treatment). The fibrinolytic function and coagulation function indexes levels of the women before and after delivery, the bleeding volume of the women in 2h and 24 hours after treatment, and the change of hemoglobin before delivery to discharge from hospital of the women in the two groups were analyzed. The clinical efficacy and the complications rate of the women were compared between the two groups. Results: The level of fibrinogen (Fib) of the women in both groups after treatment had increased significantly, the level of D-dimer (D-D) of the women in both groups after treatment had decreased significantly, and the change ranges of which of the women in group B was significantly more than that of the women in group A. The levels of prothrombin time (PT) and activated prothrombin time (APTT) of the women in both groups after treatment had increased significantly, and the level of thrombin time (TT) of the women in both groups after treatment had decreased significantly (P<0.05), but there were no significant differences in the TT and APTT levels of the women between the two groups (all P>0.05). The PT level of the women in group B was significantly lower than that of the women in group A (P<0.05). There was no significant difference in the blood loss of the women at 2h or 24h postpartum hemorrhage between the two groups. The value of Hb declined (36.25±5.25 g/L) of the women in group A was significantly higher than that (33.68±6.21 g/L) of the women in group B, and the total effective rate (95.9%) of the women in group B was significantly higher than that (79.6%) of the women in group A (P<0.05). There were 2 women with slight increase of blood pressure, and 1 woman with digestive tract reaction in group A, and there were 3 women with digestive tract reaction in group B. There was no significant difference in the total occurrence of adverse reactions (5.4% vs.7.1%) of the women between the two groups (P>0.05). Conclusion: Ergonovine maleate combined with carboprost tromethamine for treating women with postpartum hemorrhage due to uterine atony can effectively improve their fibrinolytic function with definite hemostatic effect.
