The pregnancy outcomes analysis of 19948 cycles of women who treated by different protocols of artificial insemination used donor sperm |

Abstract Objective: To explore the influence of different protocols on the pregnancy outcomes of artificial insemination used donor sperm (AID). Methods: A total of 19948 cycles of AID from January 2011 to December 2017 were retrospective analyzed, which included 14390 natural cycles and 5558 ovarian stimulation cycles (ovarian stimulation by clomiphene, tamoxifen and hMG were 812 cycles, 1229 cycles and 491 cycles, respectively). The clinical effect of women was compared among the women treated by different protocols. Results: The clinical pregnancy rate of women with ovarian stimulation cycle by tamoxifen or hMG was significant higher than that of women with natural cycle (OR1.414, 95% CI 1.254-1.595; OR 1.221, 95% CI1.073-1.388), and that of women with ovarian stimulation cycle by clomiphene was significant higher than that of women with ovarian stimulation cycle by tamoxifen (OR 1.253, 95% CI 1.062-1.478). The live birth rates of women with natural cycle was significant higher than that of women with ovarian stimulation cycles by clomiphene, tamoxifen or hMG (OR 1.175,95% CI1.025-1.346;OR 1.432,95% CI1.259-1.628;OR 1.211,95% CI 1.057-1.388), and that of women with ovarian stimulation cycle by clomiphene was significant higher than that of women with ovarian stimulation cycle by tamoxifen (OR 1.219,95% CI 1.021-1.455). The multiple pregnancy rates of women with ovarian stimulation cycles by clomiphene or hMG was significant higher than that of women with natural cycle (OR 7.828,95% CI 3.976-15.413;OR 13.941,95% CI 7.617-25.518), and that of women with ovarian stimulation cycle by clomiphene or hMG was significant higher than that of women with ovarian stimulation cycle by tamoxifen (OR 21.264,95% CI2.829159.828;OR 37.871,95% CI 5.149-278.558). Conclusion: If the women have normal ovulation function, they should be accepted AID with natural cycles. And if the women need the ovarian stimulation, clomiphene protocol would be the first choice.
