孕妇血LBP、sPLA2、IL-8水平变化及诊断胎膜早破合并宫内感染价值

摘要探讨孕妇血脂多糖结合蛋白（LBP）、分泌型磷脂酶A2（sPLA2）及白细胞介素8（IL-8）水平变化及诊断胎膜早破合并宫内感染价值。方法：回顾性收集2020年1月－2021年1月本院诊治的胎膜早破合并宫内感染患者98例（病例组），单纯胎膜早破50例（胎膜早破组）、产前检查健康孕妇60例（对照组）临床资料，分析各组血LBP、sPLA2、IL-8水平变化及诊断胎膜早破合并宫内感染价值。结果：血LBP、sPLA2、IL-8水平在对照组（70.54±20.65 ng/ml、74.58±20.41 ng/ml、7.84±0.80 pg/ml），胎膜早破组（82.56±24.59 ng/ml、121.25±26.04 ng/ml、11.48±1.38 pg/ml），病例组（107.43±30.35 ng/ml、150.22±25.41 ng/ml、15.76±1.67 pg/ml）依次升高，且病例组中未足月者高于足月者（均P＜0.05）；受试者工作特征曲线分析显示，预测胎膜早破合并宫内感染，血LBP的曲线下面积（AUC）0.797，灵敏度75.5%，特异度78.5%，截断值 89.56 ng/ml；sPLA2的AUC为0.839，灵敏度76.7%，特异度为79.9%，截断值为107.84 ng/ml；IL-8的AUC为0.995，灵敏度为85.3%，特异度为86.6%，截断值为9.15 pg/ml，3项联合检测时较单独检测特异度、准确度均较单独检测AUC 为0.998，灵敏度（91.5%）、特异度（93.1%）、准确度（95.5%）更高（P＜0.05）。结论：胎膜早破合并宫内感染孕妇血LBP、sPLA2及IL-8表达异常升高，3项联合检测对胎膜早破合并宫内感染有较高的诊断价值。
